WCNAM Enrollment Application
Download and Print out an Application.

Please print out the front and back of the application. When filling out
the application please print legibly, so that we do not make any mistakes on the
enrollment cards. When filling out the generation chart, please research
back to the late 1800's at least.
Please allow enough time for these to download
before printing.
You must have Adobe Acrobat Reader installed,
if you don't have reader please click on the following link button to download
and install to your computer. Adobe Acrobat Reader is a free program.
When ready to ship be sure to
have the following:
1. Include a self addressed
stamped envelope.
2. Application filled out front
and back.
3. Copies only of you and your
ancestors birth and death certificates.
4. Application fee of $60.00
per applicant, this fee is non refundable.
PLEASE NOTE: There is now a
$10.00 annual membership fee per person if you are admitted to the rolls. Theses
dues must be paid each year by the first month of every year.
