Lost Card?

Have you recently lost your card or was it stolen.  Please contact the office for a replacement card as soon as possible.  Here's what you'll need to get the replacement card:

• A notarized letter stating your name, birth date, social security number.
• Please include your enrollment number if known.
• A replacement fee of $10.00 for your new card
• A self addressed stamped envelope to return the card to you if submitting via mail.

Please submit to our office a request for a replacement card.

No replacement cards will be processed without all required information and processing fees.
